Effect of Regional Anesthesia Techniques on Perioperative Blood Loss in Femoral Neck Fracture Surgery

This retrospective observational study evaluates the effect of different regional anesthesia techniques on perioperative blood loss in patients undergoing femoral neck fracture surgery. Patients who received tranexamic acid were grouped by anesthesia method (lumbar plexus block, spinal anesthesia, or general anesthesia). The primary outcomes were intraoperative blood loss and the change in hemoglobin between the preoperative and postoperative periods. Secondary outcomes were postoperative pain (VAS) and total analgesic consumption during the first 24 hours.

Inclusion criteria

  • Aged 18 to 100 years
  • ASA physical status I-IV
  • Underwent femoral neck fracture surgery
  • Received tranexamic acid

Exclusion criteria

  • Allergy to local anesthetics
  • Bleeding diathesis or coagulation disorder
  • Mental disorder
  • Allergy to the drugs used
  • History of cerebrovascular disease
  • Body mass index greater than 30 kg/m2

Treatment and study plan

Primary outcomes

  1. Intraoperative blood loss

    Time frame: Intraoperative (during surgery)

    Intraoperative blood loss estimated from the volume in the aspirator container and the blood absorbed in surgical sponges

  2. Change in hemoglobin level

    Time frame: Preoperative and within 24 hours postoperatively

    Difference between preoperative and postoperative hemoglobin concentration (g/dL)

Secondary outcomes

  1. Postoperative pain score (VAS)

    Time frame: 2, 6, 12 and 24 hours postoperatively

    Postoperative pain assessed using the Visual Analog Scale (0-10)

  2. Total analgesic consumption

    Time frame: First 24 hours postoperatively

    Total amount of analgesic consumed in the first 24 postoperative hours
